Position Description

This full-time benefited position is responsible for general management of the Maternity Care program at Catholic Charities and collaboration with St. John providers and staff. This position will also provide direct case management and mentorship to clients/patients within Maternity Care at Catholic Charities in the areas of financial, spiritual, and emotional guidance. Individual consultations and collaboration with an interdisciplinary team are required.

The Maternity Care program is called to provide skilled, holistic, and compassionate prenatal care to promote healthy pregnancies and support thriving families.

Ministerial

As a ministerial representative of CCEOK, it is expected that both professional and social behavior be aligned with the ethical and moral standards of the Catholic Church
Pray daily for the needs of the poor; opportunities include personal prayer, daily Mass, and/or adoration
Willingness to learn how to articulate Catholic teaching with a commitment to its application in the job function
Execute duties in a Christ-like manner with love and compassion following the values and principles of the program, Catholic Charities and Catholic Social Teaching

Responsibilities

Provide case management: assessment, client history and community referral and follow-up
Participate in the pilot program “Lifting Moms Out of Poverty Study” with the outcome being a replicable, sustainable, measurable model, with results shared across the Catholic Charities network
Participate in the interdisciplinary team including case reviews
Envision new opportunities for outreach and growth
Coordinate schedules with St. John providers and OU for patient care and insurance enrollment
Maintain records and reports within ethical and legal guidelines using assigned database
Participate as a representative of Catholic Charities in the community at large
Maintain standards of practice through licensure, certification, or continuing education
Embrace Catholic Bioethical guidelines when working directly with clients and referral sources
Encourage and refer to Natural Family Planning classes
Work cooperatively with and train volunteers in the program
Confirm the staff is updated annually on Ethical and Religious Directives for Catholic Healthcare

Qualifications

Bachelor’s degree and minimum of 2 years case management experience preferred in social work, counseling, psychology, or related field, from an accredited program of study
Fluency in English (reading, writing, and speaking), bilingual in Spanish
Effectively communicate with individuals at all levels and from all backgrounds
Exhibit exceptional organizational skills, good interpersonal skills, work independently as well as with teams
Self-motivated and disciplined
Able to set priorities appropriately and handle multiple issues concurrently
Provide appropriate referrals for psychosocial support and intervention
Use clinical skills to manage and coordinate care of cases clinically
Assure that spiritual needs and Catholic Charities’ mission are addressed in case management activity
Promote quality improvement initiatives with current accepted clinical practice and maintain competency
Proficiency in basic computer usage including Outlook, PowerPoint, Word, and Excel
Ability to bend, stoop, lift up to 20 pounds and sit or stand for extended periods of time
