Job Summary
The Sisters of St. Joseph of Boston are seeking a compassionate and organized Congregational Healthcare Coordinator to manage the health-related needs of the congregation’s sisters. The Healthcare Coordinator will act as a liaison between the congregation, healthcare providers, and family members, ensuring that the sisters receive the highest level of care. This role is crucial in coordinating medical appointments, handling hospitalizations, managing ongoing care, and maintaining health-related records. The Congregational Healthcare Coordinator will also contribute to the overall well-being of the sisters by sharing wellness information and providing healthcare resources to the community.
Key Responsibilities
- Serve as the primary liaison for health-related matters, coordinating with individual sisters, healthcare providers, and family members.
- Accompany sisters with special medical needs to medical appointments, ensuring clear communication and follow-up on care plans.
- Communicate regularly with the President and Community Councilors, keeping them informed about significant medical issues and changes.
- Assess and coordinate the safety and viability of sisters' living situations, working closely with Community Councilors.
- Respond to all emergency and elective hospitalizations, coordinating care from admission through discharge, and working with hospitals, Bethany Health Care Center, and other facilities.
- Conduct pre-admission screenings and assessments for potential admissions to skilled nursing facilities and assist in determining care levels.
- Manage pharmacy services for the congregation, ensuring accurate and timely medication administration.
- Oversee health services for residents of St. Joseph Hall and coordinate with external providers to ensure a continuum of care.
- Provide guidance and information on health and wellness, acting as a resource person for the sisters and congregation.
- Maintain up-to-date personal health and proxy information for all sisters and assist with medical finance issues when necessary.
- Share on-call duties with a co-coordinator for weekends and holidays, ensuring 24/7 coverage for emergencies.
- Stay current with professional development by attending relevant conferences and workshops, maintaining licensure and continuing education requirements.
Qualifications
- Bachelor’s degree in Nursing, Health Administration, or a related field (required).
- License Practitioner Nurse
- 3+ years of experience in healthcare coordination, case management, or nursing, preferably in a non-profit or religious organization.
- Familiarity with healthcare regulations, discharge planning, and skilled nursing facilities.
- Strong interpersonal and communication skills, with the ability to liaise effectively between healthcare providers, sisters, and the congregation’s leadership.
- Ability to respond to medical emergencies and coordinate care in a calm and organized manner.
- Experience managing healthcare records and ensuring compliance with privacy regulations (HIPAA).
- Ability to work independently and collaboratively, with flexible availability for on-call coverage.
Job Type: Full-time
Pay: $75,000.00 - $90,000.00 per year
