Program Statement: Catholic Charities, Diocese of Cleveland is seeking a motivated, mission-driven and knowledgeable social worker to join Migration and Refugee Services. Migration and Refugee Services resettles approximately 500 refugees annually from countries including Bhutan, Burma, Sudan, Somalia, Ethiopia, Democratic Republic of Congo, Burundi, Eritrea, Iraq, Afghanistan, Syria, Pakistan, and Ukraine. We respect the dignity of each person and provide a supportive environment where each individual and culture is celebrated. Job Summary:
- Communicate and coordinate with partnering healthcare providers.
- Schedule and transport clients to initial refugee health screenings.
- Schedule advanced screenings for clients as needed.
- Accompany refugees to necessary appointments and teach refugees how to travel
unaccompanied to future appointments.
- Refer clients for ongoing medical case management as needed.
- Assist school-aged children in receiving all required immunizations for school.
- Assist eligible children in enrolling for WIC benefits and attending WIC appointments.
- Educate clients in navigation of American healthcare system including insurance, prescriptions, follow-up, etc.
- Document all activities with refugees in individual case files; ensure case files are in
compliance with agency and federal guidelines.
