Position Summary:
As a part of a multidisciplinary Residential Services Team, the Case Manager I’s role is to create initial contact and rapport with emergency shelter clients. Case Manager I’s facilitate shelter enrollment and diversion services then coordinate and participate in the development and delivery of comprehensive Housing First Individual Service Plans. Case Manager I’s facilitate housing assessments and coordinate programs and services in accordance with the needs of the clients. Utilizing concepts of Housing First and Harm Reduction modalities the Case Manager I provides supportive services for clients. Case Manager I’s play a crucial role in the success of the client moving from homelessness to home. Working hand and hand with Case Manager II’s, Case Manager I’s, coordinate “under the hood” services re: medical, mental, dental, transportation etc. Case Manager I’s advocate for clients and their needs. They also track outcomes and complete individual progress notes. Case Managers I’s are stationed within the emergency men’s shelter.
Essential Responsibilities:
- Asses and identify client’s eligibility to various housing, employment, substance use, nutritional assistance or other applicable programs available in the community and provide appropriate program guidelines to clients.
- Work closely with Case Manager II’s to create seamless transition from the homeless shelter to housing in accordance with the clients Housing First Individual Service Plan.
- Provide case management services within the emergency men’s shelter.
- Facilitate night shelter enrollment.
- Facilitate shelter diversion services.
- Maintain awareness of potential resources in the community and make proper referrals.
- Complete a range of required program related documentation utilizing a range of computer programs and data collection systems.
- Utilize electronic records to document and track all case management services and referrals.
- Complete necessary documentation to facilitate referrals to collaborative outside specialty.
- Assist clients in how to decrease systems barriers and navigate the social services network.
- Protect all confidential information, company property and electronic data. Comply with safety rules.
- Other related duties as directed.
Knowledge, Skills and Abilities:
- Must be computer literate and possess effective written and verbal communication skills.
- Ability to work independently or with a group.
- Must have a strong sense of focus and must be task-oriented and non-judgmental with a clear sense of personal boundaries.
- Must have a respect for confidentiality involving both clients and employees.
- Ability to work in a variety of settings with culturally diverse groups. Ability to be culturally sensitive and appropriate.
- Must be able to manage personal appointments and schedule, prioritize work and meet necessary deadlines.
Qualifications:
- One year of full time professional level casework experience in a social service agency preferred.
- Prior experience with homeless population required.
- Experience with chronically homeless men preferred.
- Flexible schedule to work evenings and weekends is required.
Physical Requirements:
- Primarily works in shelter dorm, office environment, but will also work throughout the agency and off site.
- Ability to regularly communicate with clients, co-workers and supervisors.
- Ability to operate and use office equipment, especially computer, keyboard and mouse.
- Ability to continuously sit and/or stand.
- Ability to walk continuously if needed and climb stairs as needed.
- May occasionally lift up to 25 pounds.
This position pays $17.00 per hour.
