Position Description
The Preferred Communities (PC) program provides intensive case management to eligible clients. This role supports vulnerable individuals still facing challenges navigating systems in achieving their self-sufficiency. This role provides services such as: long-term intensive case management, health, medical and mental interventions, social adjustment services, outreach, and engagement.
Position Responsibilities
- Provide PC grant obligated services to clients within specified time limits and Agency guidelines
- Complete intake and assessment for clients requiring intensive case management services.
- Work with clients to create and develop service plans. Track goals over the course of the program.
- Provide clients with Acculturation training, transportation orientation, and connection to services.
- Provide intensive case management to assist clients with housing, education, health support and government benefits.
- Host and attend community integration events.
- Engage with community partners to learn about resources and advocate for service provisions for PC clients.
- Conduct regular client home visits to assess their living situation and status.
- Ensure client’s case notes and case files are, accurate and complete per Agency standard.
- Submit accurate MRIS reports.
Other Duties
- Effective communication written & verbal to clients and external agencies. Represent Catholic Charities at conferences, trainings, and meetings in the community.
- Attend regular supervision meetings, case consultations, assist with the development of crisis mitigation strategies.
- Informed of the social services policies and practices in all counties and localities where the case manager is active. Seek out learning opportunities to enhance his/her capacity to assist clients.
- Willingness to uphold goals and mission of Catholic Charities Diocese of Arlington.
Qualifications & Skills
- Must have a valid driver’s license, 3 years of driving experience, safe driving record willing to transport clients in CCDA agency vehicles.
- Fluent in Microsoft Word, Office products and other relevant software.
- Effective and adaptive oral & written communication skills in English. Ability to adapt communication style to meet the needs of a culturally diverse audience.
– Bilingual in English and another language spoken by clients required: Dari, Pashtu, Arabic, Amharic, & Spanish
– Flexible to work irregular hours.
- Consistently & independently work towards self-sufficiency with all clients with minimal day-to-day supervision.
Education And Experience
- Bachelor of Social Work, Public Health, or Social Sciences from an accredited institution
- Prefer candidates have prior work experience in non-profit environment/organization with case management.
Physical Demands
While performing the duties of this job, the employee may be regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard. Specific vision requirements due to computer work and driving a motor vehicle. Light to moderate lifting up to 25 lbs. may be required.
