POSITION SUMMARY
Manages the day-to-day healthcare needs of the Sisters of Mercy in the Albany area to ensure Sister's healthcare needs are met. Provides health counseling, nursing services, and acts as health liaison with the Sister's Life Minister, physicians and/or other health care providers. Ensures sisters are treated with respect and dignity and ensures quality care as Sister's healthcare needs change. Assists in securing an appropriate placement for a Sister needing more care than her local community is able to provide.

QUALIFICATIONS
Registered Nurse licensed in the State of New York.
Strong clinical experience preferably in geriatrics and those with short and long-term healthcare needs and disabilities.
Experience in case management/care coordination.
Knowledge of and commitment to quality palliative and end-of-life care.
Ability to communicate with others with a commitment to confidentiality.
Ability to coordinate entitled services.
Valid driver's license.

RESPONSIBILITIES
Attends to the medical needs of all Albany area Sisters of Mercy, including records, medications, treatments, lab work, DME needs, doctor's visits and emergency calls as needed.
Oversees Home Health Aides for the Convent of Mercy including scheduling and performance.
Visits Sisters who are hospitalized, in a facility and at home and apprise Local Coordinator of conditions and concerns.
Provides strong advocacy for Sisters by ensuring they receive all necessary information in order to make an informed decision regarding their care.
Consults with hospital care team and House Administrator/SLMs about hospital admission and discharge plans of Sisters in the Albany area community.
Regularly observes, monitors, assesses, reports and documents Sisters' conditions and responses to treatment.
Provide emotional support and information to Sisters who are hospitalized or in a long-term care facility.
Provides physical care and support for Sisters residing in the community using appropriate healthcare resources.
Collaborates as needed with Benefits to initiate or resume medical benefits (i.e., Medicaid).
Updates House Administrator/SLMs on the health of Sisters and informs (as soon as possible) of any significant medical change in a Sister.
Accompanies Sisters to medical appointments as needed and as requested by Sisters.
OTHER FUNCTIONS
Performs other duties as assigned by Manager requiring essentially the same skills, education and efforts as documented in this position.
Participates, as appropriate, in personal professional development activities to maintain up-to-date knowledge of the requirements of the position.
May be required to work extended hours including evenings and weekends.
May be required to travel to attend meetings or accompany Sisters to medical appointments.
