General Scope of Responsibilities
Under the supervision of the Project Safe Point Program Director, this position is responsible for providing Hepatitis C Treatment Navigation services to individuals with an active HCV infection. The Hepatitis C Patient Navigator will work alongside individuals with an active HCV infection to successfully complete Hepatitis C treatment.
Duties and Responsibilities
1. Hepatitis C Screening and Confirmatory Testing
- Provide pre and post-test counseling, including thorough explanation of test results
- Perform Hepatitis C screening and confirmatory (Dried Blood Spot) testing sessions
- Work with Wadsworth Laboratory to process results of Dried Blood Spot Testing
- Manually record all data at time of screening
- Conduct HIV Screening in conjunction with HCV screening when dictated by appropriate risk factors
2. Outreach, Education and Engagement
- Conduct outreach activities to identify opportunities for Hepatitis C screening with community partners
- Identify community partners to engage individuals with untreated Hepatitis C
- Work with Project Safe Point staff to identify existing clientele in need of Hepatitis C screening
- Conduct activities that support continuity of care, including re-engaging clients in care
- Provide one on one and group health promotion activities
- Identify medical providers for Hepatitis C treatment throughout the agencies 12 county coverage area
3. Care Plan Implementation Assistance
- Facilitate assessment and care plan development
- Deliver outreach and engagement services in accordance with the service/care plan
- According to program guidelines, consult with case management staff and/or participant’s providers on progress of service/care plan services
- Document updates to electronic records, shared documents, and other formats as required by protocols, policies, and/or procedures
4. Linkage to Services
- Facilitate linkage to services
- Identify barriers to client’s accessing and completing treatment
- Provide advocacy on behalf of client with community provider’s
- Provide escort to appointments
5. Provision of Harm Reduction Services
- Syringe Exchange Services
- Risk reduction education
- Opioid Overdose Prevention Training
6. Recordkeeping
- Manually and/or electronically document activities and client/member information and interventions in client tracking systems, software programs, and other program systems.
- Collect and report on data for program evaluation to appropriate entities
- Submit required activity documentation consistently by expected deadlines.
- Track performance of program, and identify required structural changes to increase success of program. Evaluation of program should include overall testing numbers, client’s successfully receiving care for HCV, additional education and risk reduction services including, but not limited to Syringe Exchange and PrEP.
7. Facility Operations
- Maintain office in a neat, professional manner.
- Communicate to supervisor of any site-specific needs for repair or equipment.
8. Other
- Represent the agency in keeping with agency mission and standards of professionalism and ethics.
- Perform other duties as assigned by the Project Safe Point Program Director.
Designations
This position has been designated as “essential.” An essential position is one that is deemed necessary for the continued operation of the agency during periods when the organization is not operating as usual. Employees in essential positions are expected to report to work as scheduled during shifts when normal operations may not exist for the organization including, but not limited to, scheduled shifts when other offices of Catholic Charities of the Diocese of Albany are closed such as holidays, as a result of inclement weather, and during a pandemic.
As needed, the Executive Director may require that the duties of this position be performed remotely. Remote Work is a work arrangement in which supervisors direct or permit employees to perform their usual job duties away from their primary workplace, also known as the usual and customary workplace, and that job duties will be performed at an Alternate Workplace. An alternate workplace is a workplace other than the employee’s usual and customary workplace (primary workplace) and may include the employee’s home. Remote work is conditioned on the approved written plan and for the specified duration. Remote work may be discontinued at the discretion of the Executive Director or Chief Executive Officer.

Compensation
Compensation is determined by current agency policy, pending approval by the Chief Executive Officer.
This position is Salaried/Exempt and not eligible for overtime.
