Position Summary
This is an exempt position responsible for coordination and/or facilitation of services to individuals, children, and/or families to be served under Support for Trauma-Affected Refugees (STAR) program. Incumbents will utilize a holistic trauma-informed approach in the coordination of supportive services that will support the physical, emotional, and social wellbeing of the refugees and their families. The activities completed are in accordance with agency policies and procedures, federal, state, and local laws, Council of Accreditation (COA) standards, and other regulatory guidelines. The Incumbent in this position demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.
Duties & Responsibilities
Outreach
- Assists in the development and execution of outreach plans and recruitment activities to engage individuals in the community who are eligible and willing to participate in the

- Partners with ethnic community and/or faith-based organizations and providers to develop, deliver, and/or promote culturally and linguistically appropriate services for population served.

Screening & Intake
- Provides information to those in the community regarding the admission process and coordinates appointments as necessary.
- Conducts pre-screen for potential individuals within 7 days of referral to validate eligibility, giving priority to consumers that meet the priority criteria and with urgent needs or emergency situations.
- Coordinates intake activities with consumers to gather information necessary to identify critical service needs and/or determine when a more intensive service is necessary.
- Facilitates emergency assistance service for individuals served that reduce risk of homelessness and food insecurity.
- Individuals who cannot be served or cannot be served promptly are referred or connected to appropriate resources for the provision of interim services.
- Ensures intake documentation is gathered and completed timely and accurately.
- Enters intake information into agency and funding source databases within required timeframes.
- Creates consumer files.
Case Management
- Manages caseload as assigned.
- Conducts assessments that are individualized, culturally and linguistically responsive, and completed within an established timeframe.
- Provides strength-based, trauma-informed case management services either directly or by referral to include but not limited in the following areas: family strengthening, employment related services, housing, transportation, budgeting and money management, public assistance, medical and dental care, legal assistance, child care, social, cultural, recreational, and religious activities.
- Collaborates with interdisciplinary team in development of wellness service plans, the provision of information, referral and confirmed linkage to care for case management needs included in the aftercare and/or treatment plan.
- Coordinates service delivery with other community providers, bridging service gaps.
- Ensures consumers receive the appropriate advocacy support and mediate barriers to services.
- Assists with access to the full array of services to which consumers are eligible for.
- Assists individuals and families with the completion of SNAP applications, as applicable.
- Provides employment preparedness activities including but not limited to; job searches, resume writing, and interview skills.
- Conducts at minimum quarterly reviews to assess service plan implementation, progress toward achieving goals, and desired outcomes; and the continuing appropriateness of the agreed upon service goals.
- Supports consumers in the timely completion of service plan goals and objectives.
- Completes aftercare plan, follow-up activities, and closing summary.
- Actively participates in weekly multidisciplinary meetings to ensure delivery of services and review of cases.
Coaching/Teaching duties
- Facilitates evidenced based instruction and coaching that include family enrichment/strengthening, financial literacy, and/or other trainings as assigned.
- Adheres to curriculum.
- Ensures information is disseminated prior to classes/sessions.
Compliance and Reporting
- Maintains all required documentation well organized in the file.
- Completes required/ requested reports in a timely and accurate manner.
- Adheres to fidelity of evidence-based curriculums.
- Completes descriptive case notes.
- Maintain attendance record of all participants for each group.
- Records and aggregates qualitative and quantitative data as assigned.
- Ensures contract performance outcome measures are met.
- Adheres to all agency Policies and Procedures, program guidelines, funding, licensing regulations and accreditation standards.
- Monitors the compliance of safety and health precautions and monitors staff compliance according to all governing bodies’ policies and procedures.
- Meets with interdisciplinary team to discuss coordination of services for individuals served.
- Participates in the Performance Quality Improvement process.
Additional Duties
- Participates in all required or requested meetings and trainings.
- Completes other tasks as requested by supervisor.
Physical Demands
- Must possess a valid Florida Driver’s License.
- Ability to bend, stoop, stretch, squat, kneel, push, pull, crawl and sit as part of work environment.
Educational & Experiential Requirements
- Bachelor’s in social work or related area of study.
- Bilingual in English/Spanish required.
- 1 year of case management experience.
- Mental Health First Aid certified or achieved within 6 months of employment.
