JOB SUMMARY: This position is responsible for the coordination and delivery of clinical behavioral health services designed to meet the wellness and recovery needs of the consumer, including services provided to victims of violence. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumer’s family members.
ESSENTIAL FUNCTIONS
Intake:
- Intake interviews
- Preparation of diagnostic histories involving the collection, analysis and recording of significant social and personal factors
Clinical Services:
- Individual, group and family counseling and supportive counseling; this will include psych-education groups both for those directly impacted by gun violence and their families
Documentation:
- Clinical documentation should be maintained in agency EHR and should include: assessments, diagnosis form, treatment plan, progress notes, collateral contacts, group or individual notes, and termination summary.
Case Management:
- Maintenance of continuous communication with clients, including outreach for hard to engage clients
- Client advocacy services, community based counseling, coordination with referral sources
Productivity:
- Clinicians are expected to spend 65% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor.
- All schedules, statistical reports/logs, and clinical reports are to be submitted within prescribed timelines.
- Documentation should be completed in EHR within 24 hours of service delivery.
Supervision:
- Possible supervision of graduate students fulfilling field work requirement toward Master’s Degree in Social Work
SUPERVISORY REQUIREMENTS: no
MINIMUM QUALIFICATIONS: MSW from an accredited graduate school of Social Work, Counseling, or other appropriate program.
LICENSES/CERTIFICATIONS NEEDED: Clinical license required (LCSW, LPC, LAC, LSW).
