Case Manager- CRS
The Case Manager is responsible for ensuring efficient and effective delivery of the highest quality transitional and support services for clients. Services to clients are intended to aid them in improving their social, economic, cultural, or educational status. Responsibilities include providing intake functions, coordination of program activities; development and implementation of specialized programming such as job placement and retention, family budgeting, enforcement of policies and guidelines set by the program; facilitate communication between caseworkers and Program Manager to ensure the provision of quality services.
Essential Duties and Responsibilities:
- Maintain appropriate case files in accordance with HIPAA requirements and guidelines.
- Perform all intake functions; beginning file for each client, conduct assessments, referrals , and follow -ups.
- Maintain timely records using the Web-Rs database to track demographics following guidelines for participant intake, and updates as needed.
- Review applications with Program Manager to make a decision regarding the client’s eligibility.
- Ensure that all clients apply for available subsidies.
- Assist the client in setting goals and identifying their immediate needs.
- Provide information, referral, and problem-solving assistance to resident families.
- Monitor achievement of service goals.
- In conjunction with the Program Manager , provide program management, development, and expansion in conjunction with the agency's strategic plan.
- Maintain current knowledge regarding literature, trends, developments, and policy changes affecting the homeless female population.
- Produce timely reports as required.
- Attend and participate in all meetings and training as required by the agency.
- Work as a liaison with other agencies, churches, businesses, and individuals in the community to coordinate services.
- Performs other duties as assigned.
Requirements
Educations and Experience:
- Requires a Bachelor’s Degree in Social Science with a major in health or other human services discipline, or other human service fields with relevant coursework.
- Preferably shall have a minimum of two years of experience in a social work setting, case management and budgeting is essential.
- This position requires a valid Florida driver’s license, clean driving record as determined by the insurance carrier for the Diocese of Orlando.
- Requires an appreciation and respect for the Catholic Church and its teachings.
- The ability to converse, read and write in Spanish and English or other refugee-specific languages is necessary to accommodate the needs of a growing community.
