Case Manger 035
Responsibilities
- Provides case management services to individuals residing in the Ozanam Inn Shelter and PSH.
- Facilitates screening, assessment, service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers, life skills teaching, transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies and utilization of HMIS case management software.
- On-call coverage and evening hours, and must have the ability to work independently.
Requirements
- Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred.
- Experience with conflict resolution and mediation.
- Two (2) years relevant work experience and demonstrated ability to provide effective services to clients required.
- Needs to be culturally competent across all populations.
- Bilingual a plus.
- Must have driver’s license with acceptable driving history.
