Qualifications and Education Requirements:
· Two (2) years of study at a regionally accredited or NYS registered college, university, or business school; OR High School Diploma/GED and two years of experience supporting social services, employment, healthcare, or other closely related program
· Interpersonal communication skills and written skills.
· Effective time management and organizational skills.
Responsible, self-motivated and capable of managing multiple tasks.
· Working knowledge of computer programs including Microsoft Word.
DUTIES and Responsibilities
· Provide opportunities for parents to improve parenting skills and life skills to prevent placement of children or to reunite the family as soon as possible.
· Services will be provided in a family’s home or at community locations.
· Flexibility with hours is required to appropriately serve a family depending on their availability.
· A community-based, family-focused, child -centered approach will be used in the delivery of services
· Assess family needs and work with the family and caseworker to develop and establish appropriate family support outcomes and service plan goals.
· Use the COS (Circle of Security) Curriculum when appropriate.
· Maintain an average caseload of 8 families (this may vary from 5-12 depending on needs) and provide between 2-8 hours of education per family per week (this will vary depending on family progress).
· Transport families as needed to support outcomes and service plan goals.
· Supervise visits in the home as needed.
· Thoroughly document contacts into the statewide system, within 48 hours of contact.
- Attend required meetings as appropriate for program/agency and be available to testify when needed in court.
· Conduct service plan reviews every 90 days with family and supports to review goals progress and document any concerns pertaining to the child’s well-being.
· Conduct safety assessments as required.
· Assist and perform any other duties as assigned.
ADDITIONAL EXPECTATIONS
· Must have a valid NY Driver’s License, and acceptable driving record with adequate insurance.
· Must comply with all Agency policies and procedures.
· Required to understand and abide by the laws regulations, policies, and procedures that apply to the program or service that you provide.
· Required to report any known or suspected violation of laws, regulations, policies or procedures related to programs, services, agency operations, or personnel of the agency.
· Required to maintain accurate documentation including, but not limited to, purchase orders, petty cash, expenditures, mileage records, and time sheets.
