Case Manager
The Cuyahoga County Case Manager will work closely with therapists and other members to provide support, linkage to services and reinforce treatment interventions to clients in our Covenant office. Providing services that and connects to a multitude of community resources with the aim of providing stability and improving functioning. The staff member will assist with Outpatient Mental Health and School Based programs.
Responsibilities:
- Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
- Work closely with therapists to provide support, community linkages, and reinforce treatment interventions to youth and adults
- Provide clients direct service, either in the office or in the field.
- Review and monitor services, reassess needs, and initiate services which are responsive to the needs of clients; provide service coordination.
- Assist clients in developing formal and informal community supports.
- Provide advocacy on behalf of clients.
- Prepare and complete all required case recordings in an electronic medical record, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
- Attend clinical and administrative staff meetings, multi discipline team meeting as required.
- Review treatment plans and progress of cases with supervisor for continued support, advice and approval.
- Perform other duties as necessary to accomplish objectives.
