Job Summary: The Hospitalist Care Facilitator assesses, plans, implements and evaluates nursing care as directed by Hospitalist Program to maximize Hospitalist's efficiency.
Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.
Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License.
Work Experience: 5 - 7 years related experience.
Knowledge, Skills and Abilities: Working knowledge of general disease state management processes, bio-psychosocial model and clinical pathways. Effective interpersonal, written, and oral communication skills. Ability to organize and prioritize work in a effective and efficient manner Ability to utilize PC & software skills.
Essential Functions and Responsibilities: Coordinates patient discharges as ordered by the physician. Consults and collaborates with hospitalists, physicians, RN case managers, social workers, educators, and/or all other healthcare personnel, family members, as appropriate, to coordinate effective and timely patient discharge. Ensures all discharge processes are completed as appropriate. Hospitalist Care Facilitators will assist with admissions as directed. Serves as a liaison for the physician communicating with all appropriate healthcare personnel, family and patient. Hospitalist Care Facilitators will meet with hospitalists as required to discuss and collaborate patient care. Maintains the sample closet and ensures appropriate inventories/supplies as necessitated.
Decision Making: Independent judgment in planning sequence of operations and making minor decisions in a complex technical or professional field.
Working Relationship: Coordinates activities of others (does not supervise). Leads others in same work performed (does not supervise). Works directly with patients and/or customers. Works with internal customers via telephone or face to face interaction. Works with external customers via telephone or face to face interaction. Works with other healthcare professionals and staff. Works frequently with individuals at Director level or above.
