The St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager, to work in the Women’s Night Program (WNP) to provide housing-focused case management with clients. The Case Manager will engage clients; and establish relationships built on trust that collaborate with activities consistent with the clients’ housing goals. The work schedule consists of some night and weekend hours. The salary range for this case manager is $40,000 to $50,000 a year.
- Work closely with Women’s Night Program Manager
- Conduct intake on shelter referrals
- Manage caseload of up to 15-17 clients
- Provide housing focused case management for WNP clients.
- Crisis intervention with clients
- Conduct groups with clients
- Maintain accurate filing system.
- Input information into CaseWorthy, HMIS and update regularly
- Efficient in HMIS and keep records updated.
- Provide weekly updates on clients.
- Collaborate with partner agencies to address needs of clients.
- Provide initial and ongoing assessment of clients’ progress towards housing opportunities.
- Assist clients with implementing short- and long-term housing and financial stability goals using strengths and preferences as foundation to build goals.
- Develop housing stability plan with all clients with input from other treatment providers, family members and community supports, as applicable and update plan bi-weekly at minimum
- Assist clients with financial stability, including but not limited to obtaining employment, and/or government benefits.
- Accept shelter referrals on weekends.
- Maintain integrity of shelter on weekends
- Rotate on-call weeks with other case manager.
- Foster strong working relationships and provide consistent communication with community-based organizations and partner agencies to ensure coordination of services.
- Maintain database of current and community partners
- Participate in team meetings and communicate individually with team members.
- Coordinate services and generate ideas to help clients achieve housing goals.
- Transport clients to community partner appointments
- Other duties as assigned.
- Bachelor’s Degree or Certified Peer Support Specialist with at least 2 years of Case Management experience required.
- Requires Driver License (MO residents require Class E)
- Requires reliable vehicle
- Requires valid and current auto insurance.
- Experience working with mental illness and substance use and willingness to learn and gain more experience.
- Ability to handle difficult situations or people with calmness, respect, and dignity.
- Comfort and familiarity with St. Patrick Center client population
- Work a minimum of 37.5 hours a week.

- HMIS experience preferred.
- Trauma Informed Care trained preferred.
