POSITION DESCRIPTION
Job Title: Case Manager, Reception and Placement Program
Department: Community Based Service Division
Reports To: Case Management Supervisor, Reception and Placement Program
Hours: 35 hours per week flexible with evening and weekend hours
Classification: Full Time
Location: Southwest Family Service Center, 6214 Grays Avenue
Summary:
The Refugee Resettlement Case Manager is primarily responsible for providing Reception and Placement (R&P) services to new refugees, who are resettled to the Philadelphia area through a contract held with the United States Catholic Conference of Bishops (USCCB). The Case Manager will provide case management and extended cultural orientation support to ensure clients meet their resettlement goals within 90 days through the provision of services.
Principal Duties and Responsibilities:
- Ensure all core services mandated through the Department of State Cooperative Agreement are provided to clients on caseload within allocated timeframes.
- Maintain a clear understanding of the procedures of the U.S. Refugee Resettlement Program specified in the MRS Connect Database.
- Work with clients in caseload to develop strengths-based, goal-orientated service plan to support clients on their path to stability, self-sufficiency and integration.
- Provide comprehensive case management, including advocating for and coordinating access to benefits; connecting clients to medical care and education services; conducting home visits; and facilitating community navigation and the creation of social support networks.

- Exceptional interpersonal skills to work with a diverse community and build collaborative relationships with a variety of community stakeholders.
- Ability/Desire to work cooperatively as a team with other staff, the client, and client’s circle of support.
- Availability to work evenings and/or weekend hours is a must.
Education
- Bachelor’s degree in social work or related field of study
Experience
- 1 – 3 years of professional experience with vulnerable populations such as newly arrived immigrants, migrants and refugees.
Physical Requirements
- Must be able to lift up to 30 pounds.
- Must be able to stand for extended periods of time during both indoors and outdoors events.
