The Care Coordination Program Supervisor provides clinical and administrative supervision of a team if Care Manager Specialists and is responsible to oversee and ensure delivery of high quality services to qualified Medicaid recipients enrolled in the St. Joseph's Medical Center Care Coordination program. The program supervisor is responsible to lead and monitor practices to include face-to-face, telephone outreach, assessments, Plans of care, intervention, referral, and linkage to services. The Care Coordination Program Supervisor complies and maintains programmatic data reports, works with Health Home staff and health providers to ensure positive health outcomes for all clients. The supervisor reports to the Administrative Director of Care Coordination and is responsible to ensure timely and accurate data, submissions and record keeping.
Requirements
- Minimum one year supervisory experience required.

Education Requirements:
- Master's Degree in social services or behavioral health related field required.
- Professional license preferred (LMSW, LCSW, LMHC, etc)
Valid driver's license in good standing
Computer skills to include PC-based computer applications for management reporting systems, database systems, Microsoft Outlook, Word and Excel.
