POSITION SUMMARY

This case management position is assigned to the Hispanic Senior Center located at Catholic Charities’ Institute for the Hispanic Family.

This position requires the individual to provide a selective range of casework services for a broad but selected range of cases which requires a working knowledge of Agency policies and procedures together with community and environmental resources (with a special emphasis on resources for senior citizens). This person works under the supervision of a supervisor with a management classification.

GENERAL DUTIES/ RESPONSIBILITIES:

Interprets Agency services to clients and community.
Provides outreach services, with a special emphasis on local churches
Provide supportive services under appropriate supervision.
Provide advisory services.
Provides environmental services.
Develops and carries out casework plans under supervision.
Carries volume and range of work as established by Agency guidelines.
Provides coverage for activities/programming at the Hispanic Senior Center as requested by supervisor.
Does related work as required.

Successfully engage clients in services as demonstrated by client service plan completion rates, maintained or increased client functioning and client completion of service plan objectives.
Record case activity within required timeframes according to agency procedures and requirements

Assist in case management services appropriate to the personal and cultural identity and language of the client
Conduct holistic assessments of client’s service needs including biopsychosocial assessment
Assist with, in accordance with client’s needs, linkage to appropriate resources, coordination of services, and evaluation of those services in meeting the needs of the client.

Understand and follow agency policies and procedures.
Participate in ongoing supervision, consultation, staff meetings, and appropriate training.

Provide psycho education to clients
Assumes task oriented casework assignments.
Function as team member with others.
Identifies community needs.
Provides advocacy services.
PQI Functions:

Ensure the consistent accurate collection of the data
Identify patterns and trends for program
Use results of data to inform supervisor of trends that may impact services

Accept and carry out other related duties upon request

QUALIFICATIONS:

Bachelor’s Degree, preferably in a human service field and/or Associates Degree with 4 years of pertinent work experience and knowledge of Community Resources. Bi-lingual ability (Spanish/English) required.

Must work in a developmental, community and family focused manner. Must value the differences and integration of cultural attitudes, beliefs, and practices into programming.

The candidate must have high levels of personal characteristics including, but not limited to the following: enthusiasm, competency, empathy, and warmth. The candidate must also be firm but fair, encourage self-efficacy and model pro-social beliefs.

The candidate must be flexible, with a high degree of motivation, exercise good judgment, and be able to work independently.

Good Communication skills written and verbal
Knowledge of community resources
Basic Computer Skills - Proficient in Microsoft environment, notably Word, Excel and Outlook.
