Primary Functions:
Provide continuity of service through hands-on instruction/assistance to participants, as needed in areas intended to promote housing stability and independence. Ensure the safety and wellbeing of all participants. Assist and support the duties, tasks and responsibilities of the Senior Housing Case Manager as directed, as well as during his/her absence. Assists with all aspects of compliance with OMH, HUD, OTDA, and other contract and regulatory requirements.
Specific Duties:
· Assist participants with annual recertifications, preparation, submission, and renewal of
forms/documents such as social security documents, tax and rent refund forms, housing applications, Department of Social Services, etc.
· While providing coverage to case managers, you will assist participants to further develop the competencies such as, but not limited to social support systems, daily living skills, and budgeting skills, using community-based services whenever possible. Crisis interventions and de-escalation techniques as appropriate.
· Responsible for the renewal of file documents with expiration dates that include release of information (ROI) and/or consents to release information, housing quality inspection packets, and income verification.
· Be available to provide double staffing coverage to all HCM during home visits, as assigned.
· Assist HCM with the turnover of apartments for all Housing programs to ensure a timely turnover between participants.
· Report and document all incidents, first aid treatments, medical emergencies, or other crisis’ according to policy and procedures, while providing coverage for HCM.
· Maintain documentation in participant progress notes that pertain to significant events, progress toward goals, daily struggles, difficulty with other participants/neighbors, collateral contacts with participant providers, while providing coverage for HCM or when appropriate.
· Participate in scheduled team meetings maintaining updates on the status of all participants.
· Maintain clear and regular dialogue with all staff, to ensure continuity of care is maintained.
· Follow agency safety, and emergency protocols.
· Be apprised and familiar with the participant emergency contacts, hotline protocol and the location of emergency supplies at facility, utilizing police, crisis, and hotline interventions as necessary to ensure the safety and well-being of all participants.
· Complete file audits every 3 months and provide audits to assigned HCM and assigned supervisor.
· Assist with completing medication counts.
· Complete file audits for discharged clients and provide to HCM and assigned supervisor.
· Complete audits on fire drill book, reports findings to HCM and assigned supervisor.
· Assist HCM with completing filing as needed.
· Become knowledgeable of and institute the guidelines and regulations for all housing programs, per the funders’ regulations.
· Ensure that people exercise their rights and that rights are not restricted without due process.
· Ensure people are treated with dignity and respect.
· Maintain and expand skills and knowledge relevant to providing high quality support and services, keeping abreast of and following program policies and procedures.
· Appropriately communicate all pertinent information to the assigned HCM and/or assigned supervisor.
· Assist participants with transportation, as requested by Program Supervisor; including scheduled appointments or emergency needs should they arise.
· Maintain agency vehicles in a safe, clean, and functional condition, reporting any concerns or problems to an immediate supervisor.
· Attending all mandatory staff meetings, staff development training and mandatory compliance training.
· Maintain flexibility in work schedule based on needs of program participants and be willing to work overtime, non-traditional hours and/or covering additional shifts on an emergency basis as requested by a supervisor or Director.
· Perform other duties as assigned.

· Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.

· Ability to multi-task and prioritize duties.
· Willingness to foster agency, department and program wide cooperation and teamwork through use of positive/constructive communication techniques.
· Ability to maintain absolute confidentiality of all records reviewed including consumer records, employee records and billing records.
· Proficiency and experience with PC’s and Microsoft applications.
· Ability to analyze and interpret data and to handle problem resolution.
· Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
· Continuous use of a reliable, registered, and insured vehicle.
· Demonstrate commitment to Agency Mission Statement.
· Working knowledge of community resources and funding systems external to the agency.
· Demonstrated knowledge of program compliance, regulations, and requirements of funding sources, HIPAA, and with the policies of Gateways Community Living Program including the Catholic Charities.
Ability to meet the following physical requirements with or without reasonable accommodation:

QUALIFICATIONS:
EDUCATION: Highschool Diploma/GED
EXPERIENCE: Two years previous experience directly with participants in the mental health field preferred.
