Position Summary: :As a Bilingual Community Services Case Manager, you will serve those in need of pregnancy support, rent and utility assistance, human trafficking victims assistance, and/or homeless services support. You will serve clients in need by offering them a variety of resources to improve their unique situations with a path to self-sufficiency.
Essential Duties/Responsibilities::
- Provide case management services and related support to adults and children in various programs within the agency that may include but are not limited to:
- Pregnancy and Parenting Services
- Education Creates Hope and Opportunity (ECHO) Services
- Resource & Referral Services
- Human Trafficking Services
- Homeless Shelter Services
- Thrive Life Skills Course
- Communicate effectively with clients and have a solid understanding of how to work with people from diverse backgrounds
- Provides direct case management services to clients in office, home, community or mobile unit settings including assessments, case planning, advocacy, referral, crisis management, skills development and problem solving as needed.
- Assists pregnant and parenting teens to address barriers faced in completing their high school education or high school equivalency program and/or post-secondary program completion.
- Provides direct services to clients in need of emergency financial or material assistance or enrolled in self-sufficiency programming.
- Provides one on one or small group skills building or training for clients including budget review, financial literacy, banking, job skills development, job search and interviewing practice, school enrollment, FAFSA completion, etc.
- Provides case management services to residents transitioning out of the Center for the Homeless.
- Provides direct service and coordinate activities for clients impacted by disasters.
- Develops and maintains relationships with community service providers to coordinate client services.
- Promotes and represents the programs in the community and with clients.
- Attends relevant community service trainings, committee and service provider meetings.
- Ensures that all case documentation and case records are current and accurate.
- Assists in the collection of data, preparation of grants and program reports as may be needed.
- Complies with all grant, funder and audit requirements of the program.
- Perform related functions necessary to support the mission and core values of Catholic Charities.
Qualifications::
- Bachelor’s degree in social service or related field from an accredited college/university and a minimum of one year work experience in social service setting.
- Bilingual in Spanish and English required.
- Working knowledge of community service and public program providers.
- Working knowledge of CORE 40/ HSE requirements, college application & entrance requirements and FAFSA application process.
- Working knowledge of employment skills development and financial literacy.
- Ability to work independently and effectively to provide services with limited supervision.
- Ability to effectively represent the program in public situations.
- Excellent written and oral communication skills.
- Ability to travel in personal vehicle within the West Region and the Diocese of Fort Wayne South Bend, Inc. as needed.
- Computer literacy in Microsoft Office 365
Conditions Of Employment::
- Have a valid driver’s license with a clean driving record.
- If your position requires you to transport clients, staff members, or volunteers with your personal vehicle as needed, it is required that you have a reliable vehicle with personal vehicle liability insurance coverage of per person and per occurrence.
- If your position does not require you to transport clients, staff members, or volunteers with your personal vehicle, or such transportation is restricted to use of agency vehicles, your personal vehicle liability insurance coverage must be at least per person and per occurrence.
- Successful completion of federal and state background checks
- Successful completion of child welfare screening
- Successful completion of drug and Mantoux (TB) screens
