GENERAL SUMMARY
The primary responsibilities of the Intake Coordinator are to perform and oversee all resident intakes, to initiate resident familiarity with program guidelines, and to properly assess an individual’s qualifications before admission. Coordinators work closely with the Intake Manager, Motivational Track Manager, and Shelter Directors to ensure a safe and appropriate transition for each new resident to their respective program. St. Matthew’s House is a faith based 501(c)(3) charitable organization that exists to transform lives in a spiritual environment that is both compassionate and disciplined.
ESSENTIAL FUNCTIONS:
- Ensure that all intakes are completed with Matthew 25: 35-45 principles at the Ft. Myers shelter
- Assesses the appropriateness and eligibility of potential residents prior to admission
- Responsible for screening individuals to determine eligibility and managing the shelter waiting lists
- Creates and reviews resident records created at admission
- Enters accurate objective documentation into the Mission Tracker system
- Completes automated intake process through Mission Tracker
- Creates bed assignment along with resident chore and case manager
- Reviews expectations of the program to residents at admission
- Conducts property search for each new intake
- Coordinates transportation needs for new residents
- Provides excellent customer service to internal and external customers
- Works with referral agencies to schedule admissions of those being discharged
- Checks personal emails and MARS report daily and responds appropriately
- Assists the Program Staff at the Ft. Myers Shelter with various duties and shift coverage as needed
- Participates in Team meetings and projects as assigned by Intake Manager and Shelter Director
- Presents the Gospel message and prays with residents when the opportunity is available.
- Completes all job duties with St. Matthew’s House core values as the guiding principle
Requirements:
Education: High School Diploma required or equivalent. Individuals with Certification in Addiction or behavioral health preferred.
Experience: A minimum of 2 years’ experience working with individuals who are homeless or suffering from addiction.
Confidentiality: Works with highly confidential information.
Mental/Visual Demands: Flow of work requires normal mental and visual capabilities. Bi-lingual in English and Spanish preferred.
Physical Demand: Must be able to stand for a minimum of 4 hours at a time and lift at least 25 pounds. Must be able to bend or kneel to the ground.
Working Conditions: Normal office environment.
