ABOUT THIS ROLE:
St. Matthew’s House is seeking a Case Manager to provide spiritual counseling and case management to the residents in Justin’s Place Recovery Program that supports a Christ-centered approach to life transformation. The Case Manager will assist the resident in achieving self-sufficiency through advocacy; setting goals, accessing community resources and support services. Case Manager will work one-on-one with residents through case management to develop plans/goals and evaluate progress. Serves as an advocate for residents in order to obtain services-both within St. Matthew’s House and through community resources that will enable them to function independently.
WHAT YOU’LL DO:
- Provides individualized case management to assigned case load.
- Facilitates group classes in accordance with program curriculum.
- Monitors the development care plans for residents; instructs staff in completing necessary paperwork, including goals or budget forms; completes orientation checklist form and obtains release of information forms, as needed.
- Prepares and maintains case notes dealing with extenuating circumstances relating to resident issues; ensures the accuracy and completeness of same.
- Assess cases appropriately and utilize the most appropriate community resources.
- Oversees the periodical apartment, room, and bunk checks to ensure resident’s living quarters are properly maintained.
- Performs random drug screens on residents that are on their caseload.
- Coordinates required services to resolve emergency problems in crisis situations in support of continued recovery.
- In conjunction with the case management staff, oversees the referral of residents to other agencies based on a resident’s needs and in accordance with their program plan.
- Is able to complete discharge of residents with accurate information.
Requirements:
WHAT WE’RE LOOKING FOR IN YOU:
- Bachelor’s degree in social work, addiction studies, counseling, psychology, biblical studies, or related field preferred.
- State of Florida Certified Addictions Counselor (CAC) preferred.
- A minimum of one years’ experience in social work, addiction treatment or biblical counseling preferred. Faith-based non-profit experience desired.
- Knowledge of principles and practices of social work and case management techniques.
- Ability to use de-escalation and crisis intervention techniques.
- Demonstrated knowledge of substance abuse and co-occurring disorders.
- Thorough knowledge of individual and group counseling techniques and the ability to effectively facilitate groups.
- Basic understanding of mental health disorders requiring referrals.
- Understanding of motivational interviewing, brief interventions, counseling, referral, and case management process.
- A demonstrated ability to work with individuals of complex needs and diverse populations.
